Introduction
In 1867, the state of Ohio mandated that each county’s probate court maintain records of all births and deaths occurring within each county. However, there is no statewide index of Ohio birth and death records prior to 20 December 1908. The twenty-one rolls of microfilm were reproduced from the original indexes maintained AFTER 1908 by the Ohio Department of Health and the Bureau of Vital Statistics, a division of the Ohio Secretary of State’s Office. The microfilm is located in the Hayes Presidential Center’s Library Reading Room. For information on requesting photocopies of death certificates, contact the Ohio Historical Society, the organization which photocopied the records. The Ohio Historical Society maintains an Online Death Certificate Index database for the years 1913 to 1937 ( Birth records can be requested through the Ohio Department of Health, Division of Vital Statistics, P.O. Agency History
The Bureau of Vital Statistics, a division of the Ohio Secretary of State’s office, began collecting and recording birth records from each of Ohio’s counties in 1909. The Ohio Department of Health began maintaining records of all deaths occurring in Ohio’s counties the same year. The data requested varied, and may contain the following: personal and statistical particulars (gender, "color" or race, marital status, age, occupation, birthplace, father’s name and birthplace, mother’s maiden name and birthplace, medical certificate of death (date, time, cause of death, place of burial or removal, date of burial and undertaker).

Scope and Content
The microfilm is a copy of the actual INDEX of all deaths recorded throughout the state of Ohio and maintained by the Ohio Department of Health for the years 1908 to 1932, 1938 to 1944. The index contains the last name, first name, and middle initial of the deceased, the county in which the death occurred, date of death, death certificate number and the volume in which the death certificate is recorded. The index is arranged alphabetically by the last name of the deceased. The microfilm contains only the index to the Ohio birth records from 1908 to 1911. The index contains the first and last name of the infant, the father’s name, the mother’s first name as well as her maiden name, the county and town (or township) where the birth occurred, date of birth, and the volume and page number where the actual document is recorded. The index is arranged alphabetically by the last name of the infant.
